Please fill out or have parent/guardian fill the form out and return to the Executive Director. This form must be on file before the member may take place in any activities to include: rehearsal, performance, fund raisers, or travel.

To be Signed and Retuned to Executive Director. This waiver must be on file before the member may take place in any activities.
